<p>Pigment is put on the surface of the skin. A sharp object pushes that pigment into the dermis. Today that process is typically done with tiny needles bound closely together that are being very rapidly punctured into the skin and out, drawing ink in with them. It&#8217;s not a deep wound, but it is a wound and for some people that creates scar tissues, said Dr. Cyrus Chess, a Norwalk, Conn., dermatologist.</p>
<p>Chess cautions that people who have permanent makeup put on &#8212; such as lipstick or eyebrows &#8212; may not realize that they are actually being tattooed since the procedure is not performed in tattoo parlors, but in doctor&#8217;s offices or in salons. And they may never have had it clearly explained to them. Some people are delighted with the results, but others are not. Unfortunately, that&#8217;s when they may first realize the difficulty of removing it.</p>
<p>The colors typically used &#8212; reds, browns, rusts and whites &#8212; can be hard to get out with a Q-switched laser, Chess said. The inks may turn black when hit by the laser, and stay that way. Eyeliner that was put on as black to begin with may be easier to get out, but it is an extremely painful process.</p>

As far back as the Renaissance, the wealthy Medici family gave money and other assistance to the poor. More recently Jane Addams (1860 to 1935), a social activist and reformer, accelerated a long history of volunteerism in America.<span id="more-460"></span></p>
<p>I know &#8212; you already lead a busy life and barely have time, energy or resources to do for yourself, let alone for someone else. So why should you use any expendable time helping others? Here&#8217;s the answer: By doing so, you&#8217;ll get far more back than you give. Here are some of the ways you can personally benefit from volunteering:</p>
<p>It will put your own problems in perspective &#8212; It&#8217;s hard to feel sorry for yourself, or worry about your own issues, when you are busy helping someone else. In fact, there is hardly anything more therapeutic than lending a helping hand.<br />
It may increase your sense of self &#8212; When you are able to help someone else, you invariably feel better about yourself. You will feel valued and valuable as a result of your efforts.<br />
It will give you a purpose &#8212; Volunteering channels your time and energy. It gives you the satisfaction of knowing that you are doing something worthwhile and making a genuine contribution.<br />
It provides you a chance to socialize &#8212; Community service brings you in contact with others. It could be the person or group you are helping; an agency or board; or a fellow volunteer or caregiver. The act of helping adds to your social support system and increases your sense of connection to the world around you.<br />
It may be good for your health &#8212; Studies at both Cornell University and the University of Michigan indicate that people who volunteer live longer, healthier lives.<br />
Have I convinced you that giving of yourself may actually be of benefit to you? If so, how do you go about identifying a volunteer opportunity? Fortunately most communities have at least one organization that acts as a clearinghouse for volunteer positions. Here are some other ideas that may appeal to you:</p>
<p>Help out at your local library.<br />
Deliver food to the homebound or elderly.<br />
Volunteer at a hospital, senior citizen center or day care.<br />
Give your time to a homeless shelter.<br />
Act as a big sister or big brother.<br />
Assist at the humane society or at an animal shelter.<br />
If you have never volunteered your time before, here are some tips for making your experience a satisfying one:</p>
<p>Try to work directly with the people or person you are helping. Licking stamps or stuffing envelopes may be helpful, but will not provide you with the same satisfaction that &#8220;hands-on&#8221; work will.<br />
Don&#8217;t take on more than you can handle. It&#8217;s better to start slowly with a volunteer position. If you like it and can fit it into your schedule, gradually increase your involvement.</p>

<p style="text-align: justify;">A.How we maintain bone density is a complex topic, and certainly calcium plays an important part in it. The level of calcium in the body is vital to the regulation of many functions, including blood clotting, muscle movement and the transmission of signals throughout the cells of the body. The skeleton, along with the intestines and kidneys, is one of the three organ systems crucial to maintaining calcium levels in the body. Bone serves as a large reservoir for calcium, and if the level of calcium in the blood becomes too low, then the body takes calcium from the bones.</p>
<p style="text-align: justify;">A.To minimize this destruction of the bone, it&#8217;s essential to take in an adequate amount of calcium. The amount of calcium a person needs varies with age, but it is particularly important during the growing years. This requirement remains high until the late 20s or early 30s, at which time a person&#8217;s bones achieve their peak density. A person with high bone density in the late 20s has less chance of eventually developing the &#8220;bone-thinning&#8221; condition called osteoporosis than does someone with a lower bone density at that age.</p>
<p style="text-align: justify;">Later in life, as calcium is lost by increased bone resorption (the process by which the body uses the bone&#8217;s reservoirs of calcium), osteoporosis can set in. Treatment for osteoporosis is aimed at reducing the rate of bone loss.</p>
<p style="text-align: justify;">A.We obtain calcium primarily from food. However, calcium in food has a low rate of absorption. Therefore, people often need calcium supplements to maintain required levels of intake. Supplemental calcium is better absorbed when taken with meals. It is also important to take 400 units of vitamin D along with a multivitamin in addition to the calcium supplements.</p>
<p style="text-align: justify;">While adequate calcium intake may help stave off bone loss, calcium in itself has not been shown to increase bone density. However, the drug alendronate (Fosamax) has been shown to both increase bone density and help prevent the fractures that often occur with osteoporosis.</p>
<p style="text-align: justify;">There&#8217;re a lot of great and strong advantages of <a href="http://www.epharmacy-one.com/">generic prescription drugs</a>. It tends to be worse in the morning and loosen up as the day goes by. After working out I can hardly walk the next day. It feels as though the muscle or tendon is too short and doesn&#8217;t want to allow for the full flex of the foot when walking. At these times it is also very sensitive to the touch. Any ways to relieve this?<span id="more-449"></span></p>
<p style="text-align: justify;">K.G.</p>
<p style="text-align: justify;">A.It sounds as though you might have Achilles tendinitis. This disorder occurs when inflammation develops around the Achilles tendon, the tendon at the back of the ankle that attaches the muscles of the calf to the heel bone. The two major muscles of the calf, the gastrocnemius and soleus, converge and attach to the tendon about two-thirds of the way between the knee and ankle. This tendon then inserts onto the calcaneus, also known as the heel bone. With contraction of these muscles, which occurs during walking and jumping, the foot is pushed downward. At night when people sleep, these muscles relax, and the foot tends to point downward. Stiffness and pain with Achilles tendinitis is common in the morning and tends to improve as the day progresses.</p>
<p style="text-align: justify;">A.As far as treatment of Achilles tendinitis, I start with a simple approach that involves: (1) a stretching program; (2) ice after activities; and (3) anti-inflammatory medications such as ibuprofen (Motrin, Advil) as well as some of the prescription anti-inflammatory medications. Occasionally, I also recommend a shoe with a slight heel. This helps take the tension off the Achilles tendon. I ask the patient to limit activities that tend to aggravate the Achilles tendinitis.</p>
<p style="text-align: justify;">If the problem fails to resolve, then I often add a course of physical therapy. The therapist can develop a stretching program for the patient as well as use ultrasound to help reduce inflammation. Because of the risk of damage at this site from cortisone injections, these are not recommended.</p>
<p style="text-align: justify;">In a patient who fails to respond to these measures, the next step is to consider a period of immobilization with a cast. Fortunately, most cases do respond, and as symptoms subside I encourage the patient to get back into an exercise program.</p>

Should smokers take vitamins?<br />
How can smokers keep from gaining weight after they quit?<br />
I will have to admit that I was quite skeptical about the notion that smokers might be able to protect themselves against some of the hazards of smoking by taking high levels of certain vitamins. But I must say I came away from our research quite convinced that it is worthwhile for smokers to take steps to upgrade their diets. Researchers now estimate that diet is responsible for 30 percent to 40 percent of all cancers in men and 60 percent of all cancers in women. A recent report by the National Cancer Institute suggests the following guidelines to help reduce cancer risk.<br />

<p>Eat less fat.<br />
Eat more fruits, vegetables and whole grains.<br />
Ear fewer foods that are smoked, pickled or cured.<br />
Avoid food additives.<br />

Valuable Vegetables<br />
There is substantial evidence to suggest that a high, vegetable diet can protect us against cancer. A Japanese study headed by Dr. Takeshi Hitayama, chief of epidemiology at Tokyo&#8217;s National Cancer Research Institute, concluded that lifestyle factors, such as eating red meat and drinking alcohol, may increase one&#8217;s cancer risk. Subjects who smoked, ate red meat and drank alcohol, but who did not eat vegetables daily, were 2.5 times as likely to die of cancer as those who ate vegetables daily but did not drink, smoke, or eat red meat.</p>

One in 5 American high school football players suffers from a concussion annually. <p style="text-align: justify;">The sport being played is important. The criteria for return to a collision sport such as football, in which further head blows are anticipated, are different from those for a sport such as golf, in which head blows would not be anticipated. While it is best to take a conservative stance, being too hesitant about allowing an athlete to return to play may be detrimental to the athlete and/or the team.</p>
<p style="text-align: justify;">The decision to return requires consideration not only of the injury&#8217;s severity and the presence or absence of current symptoms, but also whether the athlete has sustained prior head injuries. The risk of concussion is 4 to 6 times greater for an athlete who has previously sustained a concussion.</p>

<p style="text-align: justify;"><strong>Recommendations</strong><br />
There are no universally agreed upon definitions for the various grades of concussion, nor is there unanimous agreement on the precise guidelines for return to play after a concussion. However, all guidelines agree that an athlete must not return to play until he or she recovers from all signs and symptoms of post-concussion syndrome.</p>
<p style="text-align: justify;">An athlete should never return to a contact sport while still experiencing post-concussion symptoms. The catastrophic &#8220;second-impact syndrome&#8221; may occur in athletes who have not fully recovered from an initial brain injury. Cumulative brain injury of increasing severity can occur in athletes who suffer further head trauma before recovering from an initial head injury.</p>
<p style="text-align: justify;">Post-Concussion Syndrome Signs and Symptoms<br />
Post-concussion signs and symptoms include:<br />
post-traumatic amnesia<br />
alteration of cognitive function (memory and concentration)<br />
headaches<br />
nausea<br />
vomiting<br />
photophobia<br />
balance disturbance (dizziness and vertigo)<br />
Delayed signs and symptoms of post-concussion syndrome include persistent sleep disturbance, fatigue, depression and a feeling of being slowed down or &#8220;in a fog.&#8221;</p>
<p style="text-align: justify;">All guidelines concur that athletes should be completely free of post-concussion signs and symptoms, both at rest and at exertion, and exhibit a normal neurological evaluation before being allowed to return to competition. Opinions vary on how long an individual should be free of symptoms before return to play, but all agree that the athlete must be symptom-free.</p>
